Physical Exams and Procedures

Healthcare provider(s) asked my permission for a student or trainee to do examinations or procedures on me.

Was tubal ligation / sterilization received? If yes, was the procedure explained? If yes, did you agree to the procedure?

Did the health worker ask you permission before performing vaginal examination?

My care provider always let me know the tests or scans results.

Was there a lack of informed consent for physical exam and procedures (Student allowed to do episiotomy ‘badly’)?
