Information and Consent

Your private or personal information was shared without your consent.

At anytime during your recent labor or birth did you DECLINE care offered to you or your baby? What did you decline? Why did you decline?

I felt pressure from a healthcare provider to have treatment or procedures (for example: induction or augmentation, epidural anesthesia, caesarean section).

My healthcare provider(s) accepted when I refused treatment or procedures.

When there was decision to make, I knew what all my options were.

My care provider always made sure that I knew what any tests or scans were for.

My care provider[s] always took time to check that I understood what was happening to me and my baby.

My midwife kept me informed about what was happening during labour and birth.

My care provider always made sure that I knew I had a choice about whether or not to go ahead with any tests or scans.

My doctor or midwife asked me what I wanted to do before these procedures were done.
